January 2022 COVID-19 Booster Clinic at Wheaton
Dear Wheaton Community,
We are excited to partner again with Pelmeds to host a COVID-19 booster clinic on campus and make it available to students, faculty, and staff. On Thursday, January 27, members of our community who are eligible and have not yet received a COVID-19 booster will be able to access the Pfizer or Moderna booster (your choice) in our one-day clinic located in the Trustee Conference Room in Emerson Dining Hall.
Am I eligible to participate in this booster clinic? To receive a booster, you must:
- Be at least 18 years of age,
- AND have received the Johnson & Johnson vaccination at least two months ago OR received the second dose of Pfizer or Moderna (or other WHO-approved Covid19 vaccine series) at least 5 months ago.
Please register to book your on-campus COVID-19 booster! There are three sections to this form, followed by booking your appointment. Please do not begin the form submission until you have all the necessary information available to you:
- Demographic Information
- Address Information
- Submit proof of insurance (image or pdf file)
Note that the vaccine is available to you at no cost even if you do not have insurance. Once you select a date and time, you will be directed to fill out the consent form and application. You must bring your Vaccine Card to your appointment and arrive at least 15 minutes prior to your scheduled appointment to fill out the necessary paperwork.
Walgreens and CVS stores in the local area continue to offer the COVID-19 booster, but we wanted to bring this opportunity to our community. Registration closes at noon on Wednesday, January 26 and there are only 300 appointments available, so please use this link to schedule your appointment.
